Once I have responded to your submission of the Officiating Availability Form and have confirmed that I am available to officiate your wedding, then you should fill-out the form below, in order to secure your wedding in my schedule.

So, if I have already communicated with you that I am available to officiate your wedding on your chosen date and you would like me to schedule your wedding ceremony in my planner, please fill-out the form below.

This form helps me manage my wedding schedule and serves as an easy way for you submit information to me about wedding.
